  • Are you accepting New Patients?

    Yes we are accepting new patients.  Although we have been open since 2003 and have a very large patient family, we do make availability for those new patients who may need.

  • What is the fax number?

    For both offices, the fax number is 860-886-4848.  It is an online system that will link to both locations.

  • Which phone number should I use to reach the office?

    The main number for the Norwich office is 860-886-4747.  The main number for the Mansfield office is 860-872-Foot (3668).  You may call either number and it will reach the office that is open for that day.

  • I called the office but reached a message, how do I get in touch?

    During regular business hours, if you reach an outgoing message, it is because we are occupied on another line with other patient questions.  Your message will be handled as quickly as we can.

  • What insurances do you accept?

    ALL MAJOR INSURANCES including MEDICARE and all the Medicare Advantage Plans.  We also accept workman’s compensation.

    We do NOT accept Husky or Medicaid, if it is your ONLY insurance.  But if you have another insurance as primary and the Medicaid is secondary, we do accept the primary coverage.

  • Do you accept Husky insurance?

    We do not accept Husky or Medicaid, if it is your ONLY insurance.  But if you have another insurance as primary and the Medicaid is secondary, we do accept the primary coverage.

  • Are you part of Hartford Healthcare?

    Dr. Levine is a member of the surgical staff of WIlliam Backus Hospital, which is affiliated with the Hartford Healthcare system.  Therefore, he performs his surgeries at this hospital.  But the practice is not owned by Hartford Healthcare, which allows us to provide that extra special ‘private practice’ approach that people often miss now-a-days.

  • How quickly can I have an appointment?

    Although we have a very busy schedule, for matters of urgency whether injury, infection, pain, etc, we will accommodate to bring you in relatively quickly, sometimes the same day.   If you have an urgent matter, please advise us.

  • Is there a Tolland office location, that I see listed on line?

    In July 2022, after 18 years at the Tolland location, on 200 Merrow Rd, we moved to a more suitable and better location to accommodate patient needs. Our location at 34 Professional Park Drive with Mansfield Family Practice allows us to provide more availability for our patients.   We still have the same phone number for the location.

  • I usually see Dr. Levine in the Mansfield office and I am having surgery. I know that I have to go to Backus hospital for the procedure, but may I follow up in Mansfield?

    Absolutely.  You may come to whichever location is most convenient.

  • May I come to one of the locations and then follow up in the other?

    Absolutely.  You may schedule at whichever location is most convenient for you.

  • May I send information either to register or of my previous medical records via email?

    You may send any correspondence by either regular mail or by fax, but not email.

  • Do you have someone who speaks spanish?

    It is best to bring a family member for assistance. However, Dr. Levine does understand conversational Spanish.   So we will be able to accommodate your needs either way.  If it is your first visit, please be aware that there are registration papers to complete.  So if you come without assistance, please have someone help you complete the registration papers at home and bring them with you.  You will find them on the mail page of this website.

  • My pharmacy does not have the prescription which I was told would be sent in.

    We utilize a national-online electronic prescribing system that immediately transmits your prescription to the pharmacy at the time of your visit.  Sometimes a delay is due to your pharmacy receiving the electronic transmission.  Other times, pharmacies may become inundated with so many prescriptions being sent electronically from many different sources that they are unable to complete the process, and fill your prescription.  In this case, we will RE-Send your prescription if needed.  But first speak to the pharmacy,

  • I need a refill on my prescription.

    For refill requests, please contact your pharmacy first and they will request a refill from us. 

  • Can I access my records on MY CHART?

    We do not post to My chart or similar portal.  The required involvement and time for staff members to tend to this, is used more appropriately to tend to the needs of our patients while you are here.  Certainly we are glad to provide whatever documents or records you may request if you like.

  • Do you send appointment reminders?

    We send several consecutive email reminder messages for your upcoming appointment to the email you have provided us upon your registration. 

  • Do you accept Care Credit?

    No we do not.  However, we do accept cash, check and debit cards and Mastercard and Visa.  Care Credit is basically a credit card to be used only for medical bills, but it is very similar to a visa or mastercard, in the same manner.
